Multiple Autologous Bone Marrow‐Derived CD271+ Mesenchymal Stem Cell Transplantation Overcomes Drug‐Resistant Epilepsy in Children

چکیده
There is a need among patients suffering from drug-resistant epilepsy (DRE) for more efficient and less toxic treatments. The objective of the present study was to assess the safety, feasibility, and potential efficacy of autologous bone marrow cell transplantation in pediatric patients with DRE. Two females and two males (11 months to 6 years) were enrolled and underwent a combined therapy consisting of autologous bone marrow nucleated cells (BMNCs) transplantation (intrathecal: 0.5 × 109 ; intravenous: 0.38 × 109 -1.72 × 109 ) followed by four rounds of intrathecal bone marrow mesenchymal stem cells (BMMSCs) transplantation (18.5 × 106 -40 × 106 ) every 3 months. The BMMSCs used were a unique population derived from CD271-positive cells. The neurological evaluation included magnetic resonance imaging, electroencephalography (EEG), and cognitive development assessment. The characteristics of BMMSCs were evaluated. Four intravenous and 20 intrathecal transplantations into the cerebrospinal fluid were performed. There were no adverse events, and the therapy was safe and feasible over 2 years of follow-up. The therapy resulted in neurological and cognitive improvement in all patients, including a reduction in the number of epileptic seizures (from 10 per day to 1 per week) and an absence of status epilepticus episodes (from 4 per week to 0 per week). The number of discharges on the EEG evaluation was decreased, and cognitive improvement was noted with respect to reactions to light and sound, emotions, and motor function. An analysis of the BMMSCs' characteristics revealed the expression of neurotrophic, proangiogenic, and tissue remodeling factors, and the immunomodulatory potential. Our results demonstrate the safety and feasibility of BMNCs and BMMSCs transplantations and the considerable neurological and cognitive improvement in children with DRE. Evaluation of Therapeutic Effects of Autologous Bone Marrow Mesenchymal Stem Cells to Prevent the Progression of Chronic Nephropathy in Renal Transplant
Background Chronic allograft nephropathy(CAN) is one of the most common causes of chronic and end stage renal disease. It is defined with Mainly tubular atrophy and interstitial fibrosis and no evidence of any other etiology, or functional disorder that caused at least three months after transplantation . Control of risk factors (HTN,DM,HLP, …) and limiting usage of calcineurin inhibitors...
متن کاملHarvesting of bone marrow mesenchymal stem cells from live rats and the in vitro differentiation of bone marrow mesenchymal stem cells into neuron-like cells
In the bone marrow, there are certain populations of stem cell sources with the capacity to differentiate into several different types of cells. Ideally, cell transplants would be readily obtainable, easy to expand and bank, and capable of surviving for sufficient periods of time. Bone marrow mesenchymal stem cells (BM-MSCs) possess all of these characteristics. One of the most important benefi...
متن کاملTransplantation of umbilical cord and bone marrow-derived mesenchymal stem cells in a patient with relapsing-remitting multiple sclerosis.
There is currently great interest in the use of mesenchymal stem cells as a therapy for multiple sclerosis with potential to both ameliorate inflammatory processes as well as improve regeneration and repair. Although most clinical studies have used autologous bone marrow-derived mesenchymal stem cells, other sources such as allogeneic umbilical cord-derived cells may provide a more accessible a...

Background: Renal transplantation is the best choose in the end stage renal disease (ESRD), and acute rejection and graft dysfunction remain major challenges in the worldwide, even with the advent of new immunosuppressive drugs. The novel cell-based anti-rejection treatments have been studied by using different stem cell sources. In this study, transplantation of autologous-bone-marrow-derive...

Introduction: Multiple sclerosis (MS) has been recognized as a common neurodegenerative disease that occurs after an Auto reactive T cells against myelin antigens. Demyelination and inflammation are the main features of this disease. The anti-inflammatory and neuroprotective roles of bone marrow-derived mesenchymal stem cells (BM-MSCs) have been considered as a suitable tre...
